his impotence has led to him becoming a chain smoking, inattentive, alcoholic.

Me think the spoon was the straw the broke the camel's back. Him couldn't take the nagging one second longer.
Why she didn't learn not to follow another man to the train track? Notice him put an extra rope on just in case someone else tries to save her? The train is right round the corner.
