KINGSTON, Jamaica &ndash; Usain Bolt cemented his name in athletics  history as Jamaica won the men&rsquo;s 4x100 relay in Rio to give the sprint legend the treble treble at the Olympic Games. Japan upset the fancied USA to take silver while Canada placed fourth.
