A gunman walked into the gym, turned off the lights and began firing in an aerobics area, television station WPXI reported, citing witnesses.
Collier Township police Chief Thomas Devin said nine or more persons may have been shot with a handgun.
KDKA said the killings were at an L.A. Fitness Center not far from downtown Pittsburgh and the Pittsburgh International Airport.
Allegheny County police and the medical examiner's office wouldn't confirm that to The Associated Press.
A witness told the Pittsburgh Post-Gazette newspaper she was in an exercise class when a man came in a back door and started shooting. Ashley Ogordowski said a friend of hers was among the wounded.
Gym member Lauren Dooley told the Post-Gazette she heard 12 to 15 shots.
Perry Calabro was playing racquetball inside. He said when the shooting began "everybody started running."
